Patients who are using medical insurance and will be seeing the doctor for a medical condition (red eye, pink eye, styes, allergies, trauma, etc.) will need to print, fill out and bring this form with them in addition to the Patient History Form.

If you are new to our practice, please download our Notice of Privacy Policies and Receipt of Notice of Privacy Policies. HIPPA privacy laws require that we have a signed copy of your Receipt of Notice of Privacy Policies on file. Please bring this signed form in with you on your first visit.
